Liability Waiver

Welcome to Bucks County Creative Reuse! As you know, we are an organization
that depends on the donations of others – including supplies, merchandise, financial support, and time – to serve our guests and further our mission. By signing below, you are acknowledging that you are fully responsible for your own health and well-being in all respects, including and without limitation, in connection with your ingestion of any food that may have been donated or prepared for our guests, patrons, and volunteers; your use of Bucks County Creative Reuse plastics, sewing, and other machinery; and your interaction with and use of anything else from our premises, including the facilities themselves, and fixtures, supplies, equipment, merchandise and other items here.
Bucks County Creative Reuse does not have to undertake any duty to advise you of the risks that may arise for you or to protect you from them, and, accordingly, by signing below, you are releasing
Bucks County Creative Reuse from any and all liability, accepting full accountability related to your presence and conduct here, and assuming any and all related risks. You are also agreeing that we may use any of the information you choose to provide below to contact you for any purposes.
If this is a volunteer group, the group leader is agreeing to this liability waiver on behalf of the entire group.

Bucks County Creative Reuse PHOTO RELEASE FORM:

I hereby grant Bucks County Creative Reuse permission to use my likeness in a photograph, video, or other digital media in any and all of its publications, including web-based publications, without payment or other consideration.
I understand and agree that all photos will become the property of Bucks County Creative Reuse and will not be returned.
I hereby irrevocably authorize Bucks County Creative Reuse to edit, alter, copy, exhibit, publish, or distribute these photos for any lawful purpose. In addition, I waive any right to inspect or approve the finished product wherein my likeness appears. Additionally, I waive any right to royalties or other compensation arising or related to the use of the photo.
I hereby hold harmless, release, and forever discharge Bucks County Creative Reuse from all claims, demands, and causes of action which I, my heirs, representatives, executors, administrators, or any other persons acting on my behalf or on behalf of my estate have or may have by reason of this authorization.
I HAVE READ AND UNDERSTAND THE ABOVE PHOTO RELEASE. I AFFIRM THAT I AM AT LEAST 18 YEARS OF AGE, OR, IF I AM UNDER 18 YEARS OF AGE, I HAVE OBTAINED THE REQUIRED CONSENT OF MY PARENTS/GUARDIANS.
